I am authorized to make this gift on behalf of an organizational donor (i.e., donor is a corporation, trust, partnership, LLC, association or other entity organized separately from the individuals who control it), and I am further authorized by that organizational donor to certify as follows:

- The donor is not organized under the law of a sanctioned country (Information on
